ABSTRACT

Drug-refractory epilepsy (DRE) may be treated by surgical intervention. Intracranial EEG has been widely used to localize the epileptogenic zone (EZ). Most studies of epileptic network focus on the features of EZ nodes, such as centrality and degrees. It is difficult to apply those features to the treatment of individual patients. In this study, we proposed a spatial neighbor expansion approach for EZ localization based on a neural computational model and epileptic network reconstruction. The virtual resection method was also used to validate the effectiveness of our approach. The electrocorticography (ECoG) data from 11 patients with DRE were analyzed in this study. Both interictal data and surgical resection regions were used. The results showed that the rate of consistency between the localized regions and the surgical resections in patients with good outcomes was higher than that in patients with poor outcomes. The average deviation distance of the localized region for patients with good outcomes and poor outcomes were 15 mm and 36 mm, respectively. Outcome prediction showed that the patients with poor outcomes could be improved when the brain regions localized by the proposed approach were treated. This study provides a quantitative analysis tool for patient-specific measures for potential surgical treatment of epilepsy.

ABSTRACT

ABSTRACT Vortioxetine is a multimodal antidepressant agent that modulates 5-HT receptors and inhibits the serotonin transporter. It is indicated especially in cases of major depressive disorder related to cognitive dysfunction. There are many studies investigating the effects of antidepressants on the seizure threshold and short-term epileptic activity. However, the effect of vortioxetine on epileptic seizures is not exactly known. Our aim was to investigate the effects of vortioxetine on penicillin-induced epileptiform activity. Twenty-seven Wistar rats were divided into three groups: sham-control group, positive control group (diazepam), and vortioxetine group. After a penicillin-induced epilepsy model was formed in each of the three groups of animals, 0.1 ml of saline was administered to the control group, 0.1 ml (10 mg/kg) vortioxetine was administered in the vortioxetine group, and 0.1 mL (5 mg/kg) of diazepam was administered in the positive control group, intraperitoneally. The epileptic activity records were obtained for 120 minutes after the onset of seizure. There was no significant difference in spike wave activity between the vortioxetine and diazepam groups, whereas this was significantly reduced in the vortioxetine group compared with the controls. The administration of vortioxetine at a dose of 10 mg/kg immediately after the seizure induction significantly decreased the spike frequencies of epileptiform activity compared with the control group. No significant difference was found between the vortioxetine and positive controls. This study showed that vortioxetine reduces the number of acutely-induced epileptic discharges. Vortioxetine may be an important alternative for epileptic patients with major depressive disorder-related cognitive dysfunction.


RESUMO A vortioxetina é um agente antidepressivo multimodal que modula os receptores 5HT e inibe o transportador de serotonina. Está indicada, principalmente nos casos de transtorno depressivo maior (TDM), relacionado à disfunção cognitiva. Existem muitos estudos que investigam os efeitos dos antidepressivos no limiar convulsivo e na atividade epiléptica de curto prazo. No entanto, o efeito da vortioxetina nas crises epilépticas não é exatamente conhecido. Nosso objetivo é investigar os efeitos da vortioxetina sobre a atividade epileptiforme induzida pela penicilina. Vinte e sete ratos Wistar foram divididos em três grupos, grupo controle-sham, grupo controle positivo (Diazepam) e grupo vortioxetina. Depois, 0,1 mg (10 mg / kg) de vortioxetina foi administrado no grupo vortioxetina, e 0,1 ml (5 mg / kg) / kg) de diazepam foi administrado no grupo de controle positivo intraperitonealmente. Os registros de atividade epiléptica foram obtidos durante 120 minutos após o início da convulsão. Não houve diferença significativa na atividade de pico entre o grupo de voritoxetina e diazepam, embora tenha sido significativamente reduzida no grupo de vortioxetina em comparação com os controles. A administração de vortioxetina na dose de 10 mg / kg imediatamente após a indução das convulsões diminuiu significativamente as frequências de espícula da atividade epileptiforme em comparação com o grupo controle. Nenhuma diferença significativa foi encontrada entre a vortioxetina e controles positivos. Este estudo mostrou que a vortioxetina reduz o número de descargas epilépticas agudamente induzidas. A vortioxetina pode ser uma alternativa importante para pacientes epilépticos com disfunção cognitiva relacionada à TDM.

ABSTRACT

Abstract Background Acute myeloid leukemia (AML) is a potentially fatal hematological disease. Along with disease-related factors, patient-related factors, in particular age, are a strong predictor of outcome that influence treatment decisions. Many acute myeloid leukemia risk stratification models have been developed to predict the outcome of intensive chemotherapy. However, these models did not include physical function assessments. Methods This study investigated the impact of several factors, namely the performance status, physical function and age on the short-term outcomes of intensive chemotherapy in a cohort of 50 Egyptian patients with de novo acute myeloid leukemia. Results Complete remission after intensive chemotherapy in these myeloid leukemia patients at Day 28 was 56% and the mortality rate was 12% and 34% at Day 28 and Day 60, respectively. The pretreatment Eastern Cooperative Oncology Group score was significantly correlated with outcomes on Day 28 and Day 60 (p-value = 0.041 and p-value = 0.032, respectively). There were significant correlations between the two-minute walk test and outcomes of therapy on Day 28 and 60 (p-value = 0.032 and p-value = 0.047, respectively) and between grip strength test and outcomes of therapy on Day 28 and 60 (p-value = 0.046 and p-value = 0.047 respectively). Furthermore, there was a significant correlation between chair stand test and outcome of therapy on Day 28 (p-value = 0.023). Conclusion Performance status and physical function assessments were strong predictors of outcome of intensive chemotherapy in acute myeloid leukemia and we recommend the incorporation of these variables in risk stratification models for the personalization of therapy before treating acute myeloid leukemia patients with intensive chemotherapy.

The thalamostriatal pathway is implicated in Parkinson's disease (PD); however, PD-related changes in the relationship between oscillatory activity in the centromedian-parafascicular complex (CM/Pf, or the Pf in rodents) and the dorsal striatum (DS) remain unclear. Therefore, we simultaneously recorded local field potentials (LFPs) in both the Pf and DS of hemiparkinsonian and control rats during epochs of rest or treadmill walking. The dopamine-lesioned rats showed increased LFP power in the beta band (12 Hz-35 Hz) in the Pf and DS during both epochs, but decreased LFP power in the delta (0.5 Hz-3 Hz) band in the Pf during rest epochs and in the DS during both epochs, compared to control rats. In addition, exaggerated low gamma (35 Hz-70 Hz) oscillations after dopamine loss were restricted to the Pf regardless of the behavioral state. Furthermore, enhanced synchronization of LFP oscillations was found between the Pf and DS after the dopamine lesion. Significant increases occurred in the mean coherence in both theta (3 Hz-7 Hz) and beta bands, and a significant increase was also noted in the phase coherence in the beta band between the Pf and DS during rest epochs. During the treadmill walking epochs, significant increases were found in both the alpha (7 Hz-12 Hz) and beta bands for two coherence measures. Collectively, dramatic changes in the relative LFP power and coherence in the thalamostriatal pathway may underlie the dysfunction of the basal ganglia-thalamocortical network circuits in PD, contributing to some of the motor and non-motor symptoms of the disease.

ABSTRACT

This study investigates the sensitivity and specificity of predicting epileptic seizures from intracranial electroencephalography (iEEG). A monitoring system is studied to generate an alarm upon detecting a precursor of an epileptic seizure. The iEEG traces of ten patients suffering from medically intractable epilepsy were used to build a prediction model. From the iEEG recording of each patient, power spectral densities were calculated and classified using support vector machines. The prediction results varied across patients. For seven patients, seizures were predicted with 100% sensitivity without any false alarms. One patient showed good sensitivity but lower specificity, and the other two patients showed lower sensitivity and specificity. Predictive analytics based on the spectral feature of iEEG performs well for some patients but not all. This result highlights the need for patient-specific prediction models and algorithms.

La coexistencia de esclerosis hipocampal con una lesión extrahipocampal potencialmente epileptógena, situación conocida como patología dual, traza un reto para planificar la cirugía resectiva de epilepsia farmacorresistente. Presentamos un caso de epilepsia focal farmacorresistente asociada a patología dual (esclerosis hipocampal izquierda y gliosis frontal izquierda), abordada con estimulación eléctrica cortical directa y electrocorticografía intraoperatoria, con buenos resultados a corto plazo.


The coexistence of hippocampal sclerosis with a potentially epileptogenic extra hippocampal injury is known as dual pathology, and presents a challenge to resection surgery strategies in cases of drug-resistant epilepsy. We present a case of drug-resistant focal epilepsy associated with dual pathology (left hippocampal sclerosis and left frontal gliosis), treated by means of direct cortical electrical stimulation and intraoperative electrocorticography with good short-term results.

Understanding the effects of radiation and its possible influence on the nervous system are of great clinical interest. However, there have been few electrophysiological studies on brain activity after exposure to ionizing radiation (IR). A new methodological approach regarding the assessment of the possible effects of IR on brain activity is the use of linear and nonlinear mathematical methods in the analysis of complex time series, such as brain oscillations measured using the electrocorticogram (ECoG). The objective of this study was to use linear and nonlinear mathematical methods as biomarkers of gamma radiation regarding cortical electrical activity. Adult Wistar rats were divided into 3 groups: 1 control and 2 irradiated groups, evaluated at 24 h (IR24) and 90 days (IR90) after exposure to 18 Gy of gamma radiation from a cobalt-60 radiotherapy source. The ECoG was analyzed using power spectrum methods for the calculation of the power of delta, theta, alpha and beta rhythms and by means of the α-exponent of the detrended fluctuation analysis (DFA). Using both mathematical methods it was possible to identify changes in the ECoG, and to identify significant changes in the pattern of the recording at 24 h after irradiation. Some of these changes were persistent at 90 days after exposure to IR. In particular, the theta wave using the two methods showed higher sensitivity than other waves, suggesting that it is a possible biomarker of exposure to IR.

ABSTRACT

The main shortcomings of using electrocortical stimulation (ECS) in identifying the motor functional area around the focus in neurosurgery are certainly time-consuming, possibly cerebral cortex injuring and perhaps triggering epilepsy. To solve these problems, we in our research presented an intraoperative motor cortex functional mapping based on electrocorticography (ECoG). At first, using power spectrum estimation, we analyzed the characteristic of ECoG which was related to move task, and selected Mu rhythm as the move-related feature. Then we extracted the feature from original ECoG by multi-resolution wavelet analysis. By calculating the sum value of feature in every channel and observing the distribution of these sum values, we obtained the correlation between the cortex area under the electrode and motor cortex functional area. The results showed that the distribution of the relationship between the cortex under the electrode and motor cortex functional area was almost consistent with those identified by ECS which was called as the gold-standard. It indicated that this method was basically feasible, and it just needed five minutes totally. In conclusion, ECoG-based and passive identification of motor cortical function may serve as a useful adjunct to ECS in the intraoperative mapping.

<p><b>BACKGROUND</b>To study the characters of high-frequency oscillations (HFOs) in the seizure onset zones (SOZ) and the nonseizure onset zones (NSOZ) in the electrocorticography (ECoG) of patients with neocortical epilepsy.</p><p><b>METHODS</b>Only patients with neocortical epilepsy who were seizure-free after surgery as determined with ECoG were included. We selected patients with normal magnetic resonance imaging before surgery in order to avoid the influence of HFOs by other lesions. Three minutes preictal and 10 min interictal ECoG as recorded in 39 channels in the SOZ and 256 channels in the NSOZ were analyzed. Ripples and fast ripples (FRs) were analyzed by Advanced Source Analysis software (ASA, The Netherlands). Average duration of HFOs was analyzed in SOZ and NSOZ separately.</p><p><b>RESULTS</b>For ripples, the permillage time occupied by HFOs was 0.83 in NSOZ and 1.17 in SOZ during the interictal period. During preictal period, they were 2.02 in NSOZ and 7.93 in SOZ. For FRs, the permillage time occupied by HFOs was 0.02 in NSOZ and 0.42 in SOZ during the interictal period. During preictal period, they were 0.03 in NSOZ and 2 in SOZ.</p><p><b>CONCLUSIONS</b>High-frequency oscillations are linked to SOZ in neocortical epilepsy. Our study demonstrates the prevalent occurrence of HFOs in SOZ. More and more burst of HFOs, especially FRs, means the onset of seizures.</p>

ABSTRACT

OBJETIVO: presentar nuestra experiencia en cirugía de epilepsia con electrocorticografía intraoperatoria, en 19 pacientes. MATERIAL Y MÉTODO: estudio retrospectivo basado en historias clínicas de pacientes con epilepsia lesional operados en el Sanatorio Allende, de Córdoba, entre el 1 de diciembre de 1997 y el 30 de noviembre de 2013. En esta serie hubo 14 enfermos menores de 20 años y sólo 5 mayores de esa edad. Las lesiones fueron: en 10 (52,6%) displasias corticales, en 6 (31,5%) tumores, en 2 gliosis cicatrizal y en 1 cavernoma frontal. La localización fue temporal en 4 (21%) y extratemporal en 15 (79%). Tenían epilepsia refractaria 13 (67,3%) enfermos. RESULTADOS: el tratamiento fue satisfactorio si analizamos el control de las crisis. Actualmente 14 (73,6%) están libres de crisis, sólo 4 de ellos tienen un EEG anormal, por lo cual continúan medicados. De los 5 (26,4%) enfermos que continúan con crisis, 3 tienen episodios esporádicos y tienen una sola medicación; los otros 2 tienen crisis frecuentes por lo cual reciben 3 fármacos antiepilépticos. CONCLUSIÓN: la electrocorticografía intraoperatoria nos ha permitido identificar con precisión el foco epileptógeno, que en muchos casos esta adyacente o distante de la lesión


INTRODUCTION: to present our experience in epilepsy surgery with intraoperative electrocorticography in 19 patients. MATERIAL AND METHOD: retrospective study based on clinical records of patients with epilepsy operated on between December 1997 and November 2013 in Sanatorio Allende of Córdoba. In this series there were 14 patients younger than 20 years. Included: 10 (52,6) cortical displeases, 6 (31,5%) tumours, 2 cicatricial gliosis, and 1 cavernoma. The localization was temporal in 4 (21%), and extratemporal in 15 (79%). Thirteen (67,3%) patients had medically intractable epilepsies. RESULTS: in terms of epilepsy, surgical treatment with intraoperative electrocorticography was satisfactory. At the present: 14(73%) are free of seizures; only 4 had abnormal EEG and go on with anticonvulsive medication. Five patients to remain with epilepsy, only 2 of them had frequent crisis and required three anticonvulsive drugs. CONCLUSION: the intraoperative electrocorticography permitted to identify the epileptogenic area with accuracy. This area may be situated adjacent or distant to the primary lesion

ABSTRACT

INTRODUÇÃO: Displasia cortical focal é uma das formas mais frequentes de malformações do desenvolvimento cortical, estando intimamente relacionada com epilepsia de difícil controle em crianças e adultos. Caracterizam-se por alterações histológicas, imaginológicas e eletrofisiológicas peculiares. OBJETIVO: Fazer uma revisão sucinta dos principais aspectos imaginológicos, histológicos e neurofisiológicos das displasias corticais focais. MÉTODOS: Revisão bibliografia. CONCLUSÃO: As displasias corticais focais têm características clínicas peculiares, A prevalência de epilepsia refrataria entre pacientes com displasia cortical focal é bastante elevada. A RM de crânio apresenta alterações distintas a doença, podendo em muitos casos ser normal e se correlaciona com os achados histológicos. Descargas contínuas e surtos paroxísticos de alta frequência são altamente sugestivos de epilepsia devido à displasia cortical focal.


INTRODUCTION: Focal cortical dysplasia is one of the most common ways of malformation of the cortical development where they are intimate related among hard control epilepsy on children and adults. It's characterized by peculiar histological, imaginological and electrophysiological amendment. PURPOSE: To make a succinct review of the main aspects of imaginological, histological and neurophysiologic focal cortical dysplasias. METHOD: Bibliographic review. CONCLUSION: The focal cortical dysplasias have peculiar clinical features. The prevalence of refractory epilepsy among focal cortical dysplasia patients is highly elevated. The skull magnetic resonance image (MRI) presents specific amendments to the disease that could be normal in many cases and correlates with histological results. Continuous discharges and high frequency paroxistic bursts are highly pointed as epilepsy due the Focal cortical dysplasia.
